Treating PTSD in suicidal and self-
Injuring Clients with borderline
Personality Disorder: Integrating
DBT and modified Prolonged
Exposure
Melanie Harned, Ph.D., University of Washington, Seattle is coming to
Stockholm for a two days workshop in how to treat clients with
borderline personality disorder and PTSD.
Melanie Harned work as a clinian and researcher in Behavioral
Technologi in Prof. Marsha Linehans treatment center i Seattle. She has
published many research articles and projects. She is coming for the first
time to Sweden.
A number of randomized controlled trials have documented the efficacy of Dialectical Behavior Therapy (DBT) in treating individuals with borderline personality disorder (BPD), and it is now widely used in clinical practices around the world. While DBT has been shown to be highly effective in reducing suicidal and non-suicidal self-injury, it is less effective in treating PTSD, a comorbid diagnosis that is prevalent in BPD clients. Conversely, existing PTSD treatments typically exclude suicidal and self-injuring clients, making it difficult to determine the generalizability of these treatments to high-risk individuals with BPD. This workshop will describe the DBT Prolonged Exposure protocol (a modified version of Prolonged Exposure (PE) therapy) that can be added to standard DBT to treat PTSD among suicidal and self-injuring BPD clients. Pilot data on this integrated DBT and modified PE treatment indicate that it can be implemented safely in this high-risk population, and that it is associated with significant improvements in PTSD, intentional self-injury, and other trauma-related problems (e.g., dissociation, shame, trauma-related guilt). Emphasis will be placed on determining whether suicidal and self-injuring BPD clients are ready to begin formal PTSD treatment, describing the structure and procedures of the DBT PE protocol, and addressing common difficulties encountered during PTSD treatment with this complex client population. Role plays and video clips will be utilized to illustrate various treatment strategies.
Learning Objectives:
1. Participants will learn how to determine whether suicidal and self-injuring BPD clients are ready to engage in formal PTSD treatment.
2. Participants will learn the structure and procedures of the DBT PE protocol.
3. Participants will see demonstrations of effective examples of implementing the DBT PE protocol.
4. Participants will learn strategies for addressing common difficulties encountered during PTSD treatment with high-risk BPD clients.
